What is the exact name of the form to fill out in NYS regarding the following:?
Personal items that were seized from a home for potential evidence. It turns out the items were not evidence. There is a form to request that the items be returned to the owner. Does anyone know anything about this?

If David B doesn't know about a NY form, I doubt one exists. David B is an excellent source on this site, particularly on a NY specific question. In my state, a written letter to the sheriff or the police describing the items taken and specifically asking for their return would suffice. We do not have a form.
If there is such a form, it is jurisdiction specific. Contact the police department and/or district attorney's office that is holding the no longer needed material.
