Are doctors obligated to inform you of test results?

Suppose someone has some disease, but has not contacted the doctor. Is the doctor, by law, required to call the person and inform them of the results?
No, they are not obligated. Unless, its a public health issue. Why wouldn't someone want to know the results of a test?

One would think so.however, if the person doesn't give the doctor correct contact information, he cannot contact the patient.

If someone has medical tests done, you'd think they would pick up the phone and call if they hadn't heard back in a couple of days.
In my own experience, no they do not inform you of test results unless it is something very serious. I have always had to call to get my test results. I believe if you had some serious communicable disease, they would have to inform you by law.

if something comes back on your results which is a health risk to you... most doctors will contact you ASAP for you to follow up with them..

This is called 'communication'. It is your responsibility to ensure you get your results and treatments if any. Your doctor (if he ain't a witch doctor) normally contacts their patients. Still, it is your health - not theirs.
