Changing a medical narrative can be a very serious offense. As was said it is definitely fraud. If a doctor does it they can lose near license, and my not be able to practice medicine. If a non doctor does it im sure the authorities can file charges. Changing a medical account is not something to play with. Insurance companies can get involved as well as the canon. Either way there is no positive outcome from it. Ethically, it is wrong, for obvious reason. Honesty is always the best answer.

It depends on why it was changed. Change it to make it more accurate - okay. Change it to hide malpractice - not okay, fraud.
