Emancipation in md?

I am trying to find a form to be emancipated. I work and can support myself and i have a place to live. Im just wondering if anybody knows where on earth I can find this form. My parents are willing to sign it. I just dont know where to find it.. I researched this and if I attain a note signed by my parents I can be emancipated with out going through court if anybody has any information. It would be totally well appreciated.
