12 year old needs Passport, mother does not know where biological father is?
My daughter is 12, and we need to get her a passport for a Canada trip. The problem is, i don't know where her biological father is and he have never been in her life, not since she be a baby. When we were divorced 11 years ago, we both have cohesive custody. Someone told me i will run into problems getting her one. I don't know where he's at or where he resides, can someone please help and tolerate me know if there is another way around this??
my mom had alike problem with me i think you have to walk to a notary and have something notarized.

I don't focus you're in trouble at all. If I remember correctly all you entail to do is fill out the application, provide a copy of the birth certificate and pay the money. I am pretty sure they don't ask the locality of the parents. Mine hold passed away and I got my passport. I didn't have to have extermination certificates or anything. You might need a copy of the divorce decree, but I bet not. Go to the passport bureau and get the application. If there are problems, ask the people near. They're supposed to know the answers. Maybe you could try to explain the situation to wherever you're going to get your passport. I'm sure they get these types problems adjectives the time.

Sorry but I'm pretty sure you are stuck. After 9/11 The United States is incredibly strict on its rules of leaving and entering the country. get full custody or get the police involved to find this guy so you can (get full custody)
if you are remarried, win your new partner to adopt your daughter, it worked with my friend.
im contained by the exact same situation as your daughter...except im older. im still under 18 though. i had my step dad adopt me. Do NOT rely on what "someone" say.

If I recall, it doesn't ask where the father is, just WHO he is and his birthplace.
Whew, a lot of completely wrong answers here. (As you've probably already discovered, the common passport application requires signatures of both parents.)
You need to fill out a Statement of Consent: Issuance of a Passport to a Minor Under Age 16: DS-3053.
This is used in special circumstances, one man that you are an applying parent or guardian, and the written consent of the non-applying parent or guardian cannot be obtained.
