So i got jury duty..?

and i'm claiming an exemption, but i need help with the form.
(i'm a student) i have a question about the form.

At the bottom of the form that i'm supposed to send in to the county, it says:

Subscribed and sworn to before me by ___(NAME)____

this ___(SEAL)____ day of ____, ________.

______________________________...
Notary Public in and for County


My question is, am i supposed to fill this portion out, or just mail it in blank? i've filled out all the other portions, but i don't understand this part. My mom told me I have to get it notarized, but it says nothing about that and i don't know how to do that anyway, and the people at the court office don't answer the phone! please help

DON'T FILL THAT PART OUT! That is for a notary public. You need to sign the form in the presence of a notary public who will check your ID, verify that you are the person named on the form, and then sign and seal/stamp the form for you. Most banks have a notary public on staff, but be prepared: They may charge you for notarizing the document. Here in the Detroit area, the standard charge for notarizing a document is $10.00 per document.

You are supposed to get it notarized if it has that at the bottom. Most banks have a Notary. Go to your bank and they can notarize it for you.
