one out of your area? Now this is difficult as at the moment I live in a rather sizeable City in Uk and will be moving to a small town 30 miles away... The big City hospitals are great the small town ones seem not to be as good I will be working within the large city and using a weekday address here and that means paying rent and council tax surrounded by two places....!! (no other legal option!) So will I also have to vary doctors or what I am confused ----

New rules which came in last year mechanism that you have the right to choose which hospital you want to go to for most conditions (I think Cancer and mental vigour problems are the ones which you don't have choice over!). In reality, the local healthcare trust can make it massively difficult to go to the hospital of your choice and you have to be able to exchange blows your corner.
If your doctor has connections with the big hospital, he or she can generally arrange for you to step there. The fact that you work in the city give you a local connection and a good reason for going to the metropolitan hospital a bit than the local one.
