Is it okay for a doctor to charge for filling out FMLA paperwork?

Why not?...if it takes his time he should be compensated. Are there tasks are your job you are asked to do for no pay?
WHAT PAPER WORK ALL I HAD TO GET WAS A LETTER FROM THE DOCTOR. I TOOK A MONTH OFF FOR MY DAUGHTERS SURGERY.
Why would a Dr. be filling out the paperwork to begin with? This should be done by an employer, well before the time off is actually taken.Other than that,if it were for maturnity purposes, you are allowed up to,at least 6 weeks automatically (and I could very well have my numbers incorrect),
yes and no...if you ask them while at an appoitment then no they should just do it then but if you just come by and have them do the paperwork then yes a small fee (30 to 75) depending on where you live is fair.

Answer:
Yes, a doctor's office can charge a fee for this paperwor. Often the doctor's office outsources this to an outside agency (he does this to free up himself and his staff). However, if you cannot your fee, you may wish to explain this to your doctor to see if this can be waived or paid back at a later time.
